Hi, you need to register it for the arduino ide and then it will show up in vs.
You need to read up on how to add boards to the arduino boards.txt. http://sanguino.cc/softwareforwindows
After install, upload a simple sketch using the arduino ide to be sure that you have installed it correctly

I have the sanguino board allready listed (and working) in the arduino ide and visual micro points to that arduino ide exe file.

Although i havnt played with it properly yet i can load a ardiuno pde project in vs and view all the components in solution explorer.

ok i think i'v cracked it, sanguino is appearing in drop down list!
